The hip can be approached from the front, which is known as the interior approach, from the side, which is known as the anterolateral or lateral approach, and then from the back which is known as the posterolateral approach.
The advantage of an interior approach is that is minimally invasive and involves a muscle sparring approach so that we do not damage any muscles while accessing the hip joint which is naturally positional more towards the front or anteriorly. The lateral approach is also a minimally invasive approach that involves going through the outside of the hip so that we are away from the vascular or the blood vessels to the hip.
The posterior lateral approach is the third minimally invasive approach to the hip. The utility of this approach is that it can be used to perform simple hip replacement surgeries. A hip replacement surgery typically involves addressing the ball and the socket part of the hip. The socket is more in the pelvis and the ball is in the hip bone. We typically try and remove the arthritic ball and smooth out the arthritic socket to put in metal, plastic, and ceramic pieces which comprise the new hip joint.
A hip replacement comprises a metal shell that goes into the pelvis along with a very high-grade polyethylene or plastic liner. We then have a ceramic ball that fits into a metal part that goes into the thigh bone and once the ceramic fits into the plastic that becomes a brand new hip replacement. The surgery usually takes around one to two hours. A partial hip replacement might require lesser time and in the same way, a double hip replacement surgery can take more.
AFTER A HIP REPLACEMENT SURGERY
The recovery starts right away. You will be asked to stand up and move around as long as possible. You should take care of a few things after your surgery. They are:
